Genetic causes—syndromic, monogenic, and polygenic forms—were discussed, with emerging treatments like targeted medication (e.g., setmelanotide) for some genetic cases.<br /><br />The 2023 AAP guidelines frame obesity as a disease requiring early, intensive intervention rather than watchful waiting, focusing on comprehensive care: nutritional counseling, physical activity, behavioral therapy, pharmacotherapy, and when appropriate, bariatric surgery. Social determinants of health and weight stigma are critical considerations. Practical approaches include motivational interviewing and adopting a stepwise treatment algorithm in primary care, integrating intensive health behavior therapy (IHBLT) and medications like GLP-1 receptor agonists.<br /><br />Dr. Samarthi stressed prevention starting from early childhood by promoting healthy feeding, sleep, activity, and limiting ultra-processed foods (UPFs) which constitute ~70% of children's calories and disrupt metabolic and neurobehavioral pathways, driving obesity and related complications. Strategies for reducing UPF intake in clinical settings include simple counseling, focusing on sugary beverages, and family-centered care.<br /><br />She highlighted disparities in obesity prevalence and treatment access, the importance of early screening, follow-up, and advocacy, and the need to equip primary care providers with obesity care skills given the shortage of specialists. The presentation included a success story of significant BMI reduction with multidisciplinary care.<br /><br />Overall, the webinar underscored early, evidence-based, compassionate obesity care in primary care as essential to altering lifelong health trajectories.
