Video Summary
The CSPD webinar, led by pediatric anesthesiologist Dr. Stephen Long, covered pediatric sedation safety, key regulations, and California’s Senate Bill 501 impacting sedation practices. Dr. Long explained sedation depth classifications—minimal, moderate, deep sedation, and general anesthesia—and emphasized the importance of being prepared to manage a patient who progresses to a deeper sedation level than planned. Fasting guidelines were reviewed, highlighting their critical role in sedation safety, with details on appropriate fasting times for liquids, breast milk, formulas, and meals, noting some nuances for toddlers and patients on GLP-1 drugs.

Monitoring during sedation focuses on continuous assessment of oxygenation (pulse oximetry), ventilation (capnography or stethoscope), and circulation (heart rate, rhythm, blood pressure). Dr. Long stressed that visual patient assessment is just as important as monitors due to potential monitor inaccuracies.

Risk data reveal higher adverse sedation events outside operating rooms, underscoring the need for strict adherence to safety protocols. Senate Bill 501, effective 2022, mandates sedation permits and age-specific endorsements for pediatric patients under 13 and/or 7 years, with required provider numbers and PALS certification depending on sedation depth. Equipment and drug requirements vary by sedation level, with strict limits on drug combinations for minimal sedation.

During Q&A, Dr. Long advised giving oral sedatives with clear liquids, addressed parental anxiety about in-office anesthesia, discussed indications for airway adjuncts, and clarified sedation permits and safety practices. He emphasized following guidelines closely for safe sedation and acknowledged grey areas in the regulations requiring clinical judgment. The webinar series will continue with a focus on emergency preparedness.
