The use of fluoride supplements and topical fluoride therapy in preschool children is complex and controversial. Since the introduction of water fluoridation, fluoride supplements, and topical fluoride therapies in the late 1940s, the mechanisms of actions and dosages have been debated and have evolved. Fluoride varnish have become more prevalent for office treatment because of the safety of premeasured doses, better acceptance by children and evidence of effectiveness from clinical trials. Fluoride-metal compounds in toothpastes and professional topical treatments are bringing new mechanisms to fluoride therapies. Overlaying both issues of topical fluoride therapy and fluoride supplement use is the current focus on caries risk and cost versus benefit. This talk addresses both systemic and topical fluoride therapy in the context of the changes in exposure to systemic fluoride, benefits in an era of less caries, and recent changes in clinical guidelines.
